Over the last 6 weeks, a 35-year-old nurse has developed progressive difficulty getting out of chairs and climbing stairs. She can no longer get in and out of the bathtub. She has no muscle pain and takes no regular medications. She does not use alcohol and does not smoke cigarettes. On examination, she has a purplish rash that involves both eyelids. Explanation:
-It is simply polymyositis with Dermatomyositis (symmetrical leg muscle weakness with rash). -Symptoms: leg muscle weakness that might lead to hip and shoulder weakness, dysphagia or dysphonia. The rash is photosensitive rash (V-sign rash) or even heliotrope rash in the eyelids and Gottron papules (mechanic hands) -dermatomyositis (antibodies-driven autoimmune disease lead to PERIMYSIAL inflammation and atrophy, resulting in muscle tissue damage). -polymyositis (T-cell/CD8 autoimmune disease lead to ENDOMYSIAL inflammation so there would be no cutaneous manifestation if alone because it includes only the inner layer of the muscle). *Investigations - Blood Work: CK, CK-MB, aldolase, LDH, - autoantibodies: ANA, Anti-Ro, SRP, Anti-MI2 - muscle biopsy: diagnostic gold standard to differentiate between "inflammatory and non-inflammatory myopathies". - Treatment: systemic corticosteroids up to 4 weeks (1st line), anti-malarial drugs, and non-steroidal immunosuppresive drugs like methotrexate and azathioprine (usually the immunosuppresive is for refractory periods and recurrence).
